Once you have filled out the waiver form, please see a staff member at the front desk to complete the check in process.
The Gravity Vault Waiver Form
Every participant in our facility must have a completed waiver form.
If the participant is under 18 years of age, the waiver form must be completed by a parent or legal guardian.
